General Notes

The data element I10_ORPROC indicates whether a major operating room procedure was reported on the discharge record. The criteria for identifying an operating room procedure has changed over time.

I10_ORPROC is only available for some databases and data years.

Beginning in Data Year 2019

Beginning in data year 2019, the presence of operating room procedures reported on a discharge record was determined using the AHRQ Quality Indicator format for operating room procedures. For a complete list of the operating room procedure codes used, see PSI Appendix A - Operating Room Procedure Codes of the Patient Safety Indicators Technical Specifications on the AHRQ Quality Indicators website.

If at least one procedure on the record is an operating room procedure, then I10_ORPROC is set to 1.

From fourth quarter 2015 to Data Year 2018

Prior to data year 2019, the presence of operating room procedures reported on a discharge record was determined using the procedure classes for ICD-10-PCS (beta version). Each ICD-10-PCS procedure code on the record is classified into one of four procedure classes:

  • Minor Diagnostic: Non-operating room procedures that are diagnostic (e.g., BW2800Z CT scan of head using high osmolar contrast)
  • Minor Therapeutic - Non-operating room procedures that are therapeutic (e.g., 079030Z drainage of head lymphatic, percutaneous approach)
  • Major Diagnostic - All procedures considered valid operating room procedures by the Diagnosis Related Group (DRG) grouper and that are performed for diagnostic reasons (e.g., 00B00ZX excision of brain, open approach, diagnostic)
  • Major Therapeutic - All procedures considered valid operating room procedures by the Diagnosis Related Group (DRG) grouper and that are performed for therapeutic reasons (e.g., 021008W bypass coronary artery, one artery from aorta with zooplastic tissue, open approach).

If at least one major diagnostic or major therapeutic procedure is included on the record, then I10_ORPROC is set to 1. More information on Procedure Class is available under Tools & Software on the HCUP-US website.

Prior to fourth quarter 2015

For data prior to the fourth quarter of 2015, the major operating room procedure indicator is stored in the data element ORPROC to indicate the use of the ICD-9-CM coding system.
